Arkansas's agency and claim process
Arkansas claims are filed with the Workers' Compensation Commission. Under Ark. Code 11-9-702, if no bona fide hearing request is made within six months after a claim is filed, the claim can be dismissed without prejudice to refiling within the limitation period.

Report on the commission's form
Under Ark. Code 11-9-701, unless the injury is made known to the employer immediately or the employee can't report it, the employee reports on a commission-approved form, and the employer isn't responsible for benefits before it receives that report. Failing to give notice doesn't bar a claim if the employer knew of the injury.
Two-year filing limit
11-9-702 bars a claim not filed with the commission within two years of the compensable injury. If benefits were paid, a claim for additional compensation must be filed within one year of the last payment or two years of the injury, whichever is greater, and a latent injury doesn't toll these periods.
